On your first visit...
to our office you will be asked to fill out our intake form, or you can do it online by clicking here.  To help us put together the best possible treatment and payment plans for each individual, there will then be a consultation between you and the doctor in which he will go over the information you have provided and ask any extra questions that may be needed.  After Dr. Castle has obtained all the information he needs, he will then proceed with a physical exam, any necessary x-ray imaging and treatment.  You will then be asked to schedule an appointment for the next day in order to receive the report of findings.
